RESUMEN Objetivo: Describir el modelo de gestión del cuidado de enfermería en servicios hospitalarios. Metodología: La metodología desarrollada en este estudio se basó en la perspectiva cuantitativa, utilizando tipologías documental-bibliográficas que ayudan a establecer el análisis del objeto de investigación. El material abordado se conformó por tesis de grado, investigaciones científicas, trabajos arbitrados y, con ello, descubrir características y relaciones entre los elementos. En conclusión: El rol de enfermería en el desarrollo de los modelos de gestión del cuidado en los servicios hospitalarios, es de gran importancia ya que establecen los mecanismos de comunicación para su implementación en conjunto con el resto del equipo de salud, que hace vida en los centros hospitalarios.

ABSTRACT

As a new type of medical service project, "Internet + nursing service" has become a new trend in the development of nursing. Ningbo area started the exploration of "Internet + nursing service" as early as the end of 2015, and gradually formed the Ningbo model of "government-led, regionalized layout, and online and offline collaborative services, medical professional support, market-based operation". And made the innovative practice of "medical insurance pay at home". By the end of December 2021, there were 6 200 registered nurses on the platform, and a total of 20 993 door-to-door services were provided. 197 medical institutions settled on the platform and carried out 49 home service projects. This paper introduced the exploration experience and practical results of "Internet + nursing service" in Ningbo, and provided reference and reference for the advancement of this work.

Introducción: La preocupación por el entorno laboral en enfermería se ha hecho aun mayor desde la pandemia Covid-19, que afectó la cantidad y calidad del personal de enfermería alrededor del mundo. Objetivo. Describir el entorno laboral de los enfermeros que desempeñan funciones asistenciales en un hospital de Bogotá, Colombia. Método: Estudio descriptivo exploratorio, con un componente correlacional. Se remitió el cuestionario ENLASA a todos los enfermeros asistenciales del hospital objeto de estudio a través de RedCapÒ. Resultados. El cuestionario completo fue respondido por el 50% de los enfermeros asistenciales (49 enfermeros) de la institución e invitados a participar en el estudio. El 57,1% de los respondientes consideró el entorno laboral no saludable. La dimensión de elementos estructurales se consideró no saludable (64,49/100), y la de procesos organizacionales como saludable (71,18/100). Las subdimensiones no saludables fueron, en estructura, la planeación del recurso de enfermería, (54,66/100) y políticas de recursos humanos (58,91/100) y en procesos, la interacción enfermero-organización (63,10/100) y autonomía (66,07/100). No se encontró asociación entre las variables sociodemográficas y el entorno laboral de enfermería. La única asociación estadísticamente significativa entre entorno laboral y resultados fue la relativa a la motivación para trabajar. (Phi 0,362, p=0,040). Conclusiones. Se sugiere intervenir las dimensiones y subdimensiones identificadas como no saludables, y continuar monitoreando este fenómeno.


Introduction: concern for the work environment in nursing has become even greater since the COVID-19 pandemic, which affected the quantity and quality of nursing personnel around the world. Objective: to describe the work environment of nurses performing care functions in a hospital in Bogota, Colombia. Method: exploratory descriptive study. The Enlasa -Nursing Questionnaire was sent to all the nurses in the hospital under study through RedCap. Results: 50% of the nurses (49 nurses) of the institution invited to participate in the study answered the complete questionnaire. The work environment was considered unhealthy by 57.1% of the respondents. The dimension of structural elements was seen as unhealthy (64.49/100), and that of organizational processes as healthy (71.18/100). The sub-dimensions considered unhealthy in structure were nursing resource planning (54.66/100) and human resources policies (58.91/100). With respect to processes were nurse-organization interaction (63.10/100) and autonomy (66.07/100). No association was found between sociodemographic variables and nursing work environment. The only statistically significant association between work environment and results was the one related to motivation to work (Phi 0.362, p=0.040). Conclusions: it is suggested to intervene in the dimensions and subdimensions identified as unhealthy and to continue monitoring this phenomenon.


Introdução: a preocupação pelo ambiente de trabalho dos enfermeiros tornou-se ainda maior desde a pandemia de COVID-19, que afetou a quantidade e qualidade do pessoal de enfermagem em todo o mundo. Objetivo: descrever o ambiente de trabalho dos enfermeiros que desenvolvem funções assistenciais em um hospital de Bogotá, Colômbia. Método: estudo descritivo exploratório. O questionário Enlasa-Enfermagem foi enviado a todos os enfermeiros assistenciais do hospital objeto de estudo através de RedCap®. Resultados: 50% dos enfermeiros assistenciais (49 enfermeiros) da instituição convidados a participar do estudo respondeu ao questionário completo. 57,1% dos entrevistados considerou o ambiente laboral não saudável. Conclusões: sugere-se intervir nas dimensões e subdimensões identificadas como não saudáveis e continuar monitorando esse fenômeno.

Objetivo: Analisar o impacto da mudança assistencial na prevenção de IRAS em uma Unidade de Terapia Intensiva Adulto de um Hospital Universitário do Paraná. Métodos: estudo epidemiológico, quantitativo, realizado em uma UTI do Paraná. Coleta de dados ocorreu em 2020. Para a intervenção, foi realizado e validado um Fast Checklist. Resultados: A média de entradas no triênio foi de 40,83, média de dias de uso de dispositivos de 256 e a média de permanência de 6,3 dias. No biênio, foi respectivamente, 42,33 (p=0,504), 284,12 (p=0,005), 6,90 (p=0,130). A incidência de infecções, no triênio obteve-se uma média de 8, com taxa 20,42 e densidade de incidência de 32,54. Após, média de 4,91 (p=0,000), taxa de 13,15 (p=0,003) e densidade de incidência de 18,41 (p=0,000). Conclusão: as mudanças propostas e validadas tiveram grande impacto na assistência na UTI estudada, reduzindo as infecções, custos, óbitos e salvando vidas.

Methods: epidemiological, quantitative study, carried out in an ICU in Paraná. Data collection took place in 2020. For the intervention, a Fast Checklist was performed and validated. Results: The average number of entries in the triennium was 40.83, the average number of days of device use was 256 and the average stay was 6.3 days. In the biennium, it was, respectively, 42.33 (p=0.504), 284.12 (p=0.005), 6.90 (p=0.130). The incidence of infections in the three-year period was an average of 8, with a rate of 20.42 and an incidence density of 32.54. Afterwards, mean of 4.91 (p=0.000), rate of 13.15 (p=0.003) and incidence density of 18.41 (p=0.000). Conclusion: the proposed and validated changes had a great impact on care in the studied ICU, reducing infections, costs, deaths and saving lives.

La incidencia de contagios en el trienio fue de media 8, con una tasa de 20,42 y una densidad de incidencia de 32,54. Posteriormente, media de 4,91 (p=0,000), tasa de 13,15 (p=0,003) y densidad de incidencia de 18,41 (p=0,000). Conclusión: los cambios propuestos y validados tuvieron gran impacto en la atención en la UTI estudiada, reduciendo infecciones, costos, muertes y salvando vidas.

ABSTRACT

【Objective:】 To investigate the current situation of attitudes and training needs of hospice care among nursing staff in Hainan Province, and analyze the influencing factors, so as to provide basis for formulating scientific and standardized hospice care training programs for nursing staff. 【Methods:】 From October to December in 2021, a total of 1 819 nursing staff from 45 general hospitals, specialized hospitals and nursing homes in 14 cities and counties of Hainan province were selected by convenience sampling method to conduct the questionnaire on hospice care attitude and training needs. 【Results:】 1 789 valid questionnaires were collected with effective recovery of 98.35%. Nursing staff hospice care attitude score was (88.13±12.10) points, the standard score was 70.4 points, hospice care attitude was at medium level. Nursing staff training needs score was (26.96±5.16) points, the standard score was 89.87, the needs of each dimension and item were higher than 75.1%, the training needs were at high level. Professional title, currently engaged in hospice care services, and willingness to engage in hospice care services were the influencing factors of hospice care training needs (P<0.05), and the hospital, have witnessed the death of dying patients or relatives, willingness to engage in hospice care services were the main influencing factors of nurses’ attitude toward hospice care (P<0.05). 【Conclusion:】 At present, nursing staff in Hainan Province held a moderate attitude towards hospice care, and had a high demand for training. The major obstacles for nursing staff to engage in hospice care at present were high stress in the face of dying patients and symptom management. Nursing managers should combine the current situation, pay attention to stimulate nurses’ intrinsic motivation, strengthen the training of nursing staff’s concept of hospice nursing, construct a diversified training system, and carry out specialized and modular skill practice and theoretical training to improve nursing staff’s hospice care ability.

ABSTRACT

Objective:To explore the current practice of head nurses′ human caring for patients at home and abroad, and integrate those effective measures and effect evaluation methods, so as to provide reference for nursing administrators.Methods:A framework was built on the scope review method proposed by Arksey and O′Malley, and such search terms as head nurse/nursing administrator, human caring/care/human-based, sick person/patient, nursing supervisory/charge nurse/head nurse/nurse administrator/nurse manager/nurse executive, empathy/care/compassion, patient/client were used. CNKI, Wanfang database, VIP, Chinese Medical Association Journal Full-text Database, Medical Knowledge Network (PubMed, Elsevier, Springer, Wiley, OVID, EBSCO) and the Cochrane Library were searched from their initiation to November 29, 2022. Two researchers independently screened and extracted basic characteristics of the literature, as well as the measures used by the head nurses to implement human caring for patients and the effect evaluation tools.Results:A total of 57 articles were included. This paper reviewed the measures of human caring for patients at both levels of head nurses as direct caregivers and as organizers.The measures at the level of direct caregivers included implementing human caring in their ward rounds, creating a caring atmosphere, setting up a head nurse reception day, interviewing the care needs of patients and their families, innovating working methods based on the perspective of human caring, and caring communication with patients and their families; measures at the level of organizers included building a nursing human caring mode with specialist characteristics, building a human caring mode for different patient groups, strengthening the training of nurses′ human caring ability and literacy, building a caring environment and atmosphere, simplifying the nursing work process, and establishing a continuous and diversified nurse-patient communication mode, continuing human caring for discharged patients, organizing participation of nurses in social practices of human caring, setting up caring posts, and conducting care supervision and quality control. Patient satisfaction survey was used to evaluate the practical effects of human caring, but the evaluation objects were nurses or nursing services.Conclusions:Head nurses play an important role in the implementation of human caring, and a variety of measures can be taken to directly or indirectly implement human caring for patients. It is suggested to build more human caring modes to cover more specialties and patient groups, and improve the patient satisfaction evaluation tools with head nurses as the evaluation object.

ABSTRACT

Objective:To analyze the role of wechat combined with SBAR communication in emergency nursing management.Methods:From December 2019 to December 2020, 100 cases of patients in the emergency department of No.904 Hospital, Joint Logistics Support Force of PLA were randomly selected and divided into two groups according to the computer random table method, 50 cases in the control group were treated with routine nursing management, 50 cases in the study group were treated with Wechat combined with SBAR communication mode for management. The goal achievement, nursing service quality and nursing satisfaction of the two groups were compared.Results:The score of Nurse′s Performancefor Goal Attainment (NPGA) in the study group was 196.32 ± 20.36, which was higher than (165.24 ± 18.14) of the control group ( t=8.06, P<0.05), and the score of Servqual evaluation questionnaire (94.32 ± 3.36) was higher than (89.24 ± 3.14) of the control group ( t= 7.81, P<0.05). The difference of nursing satisfaction between the two groups was significant, the study group (92.00%, 46/50) was higher than (64.00%, 32/50) of the control group ( χ2=11.42, P<0.05). Conclusions:In the emergency nursing work, using wechat combined with SBAR communication management can effectively improve the quality of nursing service, improve work efficiency, and increase patients′ recognition of nursing staff.

ABSTRACT

Objective:To explore the management experience of clinical nursing managers and to provide reference for the implementation of the reform of DRGs-PPS.Methods:15 clinical nursing managers were surveyed in an in-depth semi-structured interview by phenomenological research methods, and the data were analyzed by Colaizzi 7 analysis method.Results:Four themes were extracted: the positive effect of the reform, the value reflection of nursing work, the promotion and obstacles of nursing management during DRGS implementation.Conclusions:Clinical nursing managers, there are positive experience as well as negative experience for the implementation of the reform of DRGs-PPS, so we need to constantly improve the staff′s cognition, to maximize the value of nursing, draw support from policy guidance and third-party building platforms, optimizes the allocation of human resources, strengthen the management of consumables, promote nursing quality and cost effective integration and effective supervision, and promote healthy development of the DRGs-PPS.

ABSTRACT

This article summarized the overview of diagnosis related groups (DRGs), the necessity of comprehensively popularizing and applying DRGs in specialized hospitals and general hospitals, the different methods and effects of nursing human resource allocation based on DRGs in specialized hospitals and general hospitals at home and abroad, and analyzed the different challenges and opportunities faced by DRGs in the implementation of human resource allocation in two types of hospitals. According to the types and characteristics of hospitals, this paper put forward some corresponding suggestions and prospects for the future, such as intelligent human resource prediction system and the construction of information sharing platform, so as to provide reference for the comprehensive promotion of DRGs in different types of hospitals in China.

ABSTRACT

Objective:To construct endocrinology nursing subspecialty model and explore its clinical effect.Methods:In December 2018, the organization structure of endocrinology nursing subspecialty was constructed in the Drum Tower Hospital Affiliated of Nanjing University Medical School and applied in clinic. In this model, the data of 2018 were taken as the data before application and the data of 2020 were taken as the data after application. The comprehensive ability of nurses, nurse satisfaction, related nursing workload and scientific research ability of nurses were compared before and after the application of the model.Results:After the application of subspecialty nursing mode, nurses′ comprehensive ability score was (92.00 ± 2.36) points. Compared with (84.25 ± 3.24) points before implementation, the difference was statistically significant ( t=-9.46, P<0.01); nurses′ satisfaction evaluations including specialty development (7.92 ± 1.41), self-quality improvement (8.00 ± 1.69), work pressure (6.42 ± 2.67), salary and welfare (3.96 ± 0.85), compared with (5.79 ± 2.31), (6.17 ± 2.82), (8.33 ± 1.50), (2.88 ± 1.59) before implementation, the difference was statistically significant ( t values were -3.86--2.73, all P<0.05). The annual workload of related nursing increased and the scientific research ability of nurses was improved. Conclusions:The application of endocrinology nursing subspecialty mode is beneficial to improve nurses′ comprehensive ability of clinical work, improve the level of specialized nursing, improve the quality of nursing service and promote the improvement of economic benefits, which is worthy of clinical promotion.

ABSTRACT

Objective:To investigate the effect of 1M3S nursing management mode combined with transcatheter arterial chemoembolization (TACE) on intestinal microecological distribution in patients with primary liver cancer.Methods:A total of 115 patients with primary liver cancer in Hai′an people′s Hospital from January 2017 to January 2020 were enrolled. Patients were divided into two groups according to the admission time. Patients ( n=56) receiving routine nursing care from January 2017 to December 2018 were set as control group, patients ( n=59) receiving 1M3S nursing management from January 2019 to January 2020 were set as observation group. Another 34 healthy individuals were set as healthy group from January 2017 to January 2020 in Hai′an People′s Hospital. The general data were collected in all three groups, and the serum levels of endotoxin (ET), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected. Fecal samples were collected, and 16S rDNA sequencing method was used to analyze the fecal flora structure and species relative abundance among groups, and alpha diversity was analyzed. Results:At the level of phylum, the dominant phylum of the three groups were Bacteroidetes, Proteobacteria and Firmicutes. After TACE, the ET, ALT and AST levels were (9.67±2.12) ng/L, (53.24±8.47) U/L, (55.48±8.15) U/L in the control group, (4.36±2.15) ng/L, (45.31±8.36) U/L, (47.25±8.21) U/L in the observation group ( t value was 13.328, 5.052, 5.392, P<0.05). Compared with the control group, there was an increase in the relative abundance percentage of Firmicutes( t value was 16.426, P<0.01) and Lachnospiraceae in the observation group ( t value was 4.527, P<0.01), and a decrease in the relative abundance percentage of Proteobacteria ( t value was 8.462, P<0.001) after intervention. Conclusions:TACE can affect the intestinal bacteria in patients with primary liver cancer, resulting in a decrease in the relative abundance of Proteobacteria, Lachnospiraceae, and an increase in the relative abundance of Firmicutes, while application of 1M3S nursing management mode can effectively reduce the level of endotoxin, improve liver function, and reduce the imbalance of intestinal flora caused by TACE.

Objective:In order to analyze the current research status of handover shift in nursing management, summarize, analyze and judge the existing literature, in order to provide reference for clinical nursing practice.Methods:Through literature review, it is planned to review the current situation, shortcomings and future development of nursing handover classes.Results:The handover process was generally divided into four stages, of which SBAR was the best practice tool for handing over key information. For the performance of handover shifts, NASR, PVNC-BR, HES and Handoff CEX were often used to evaluate the performance of shifts, and for the results of shifts, evaluations were mostly conducted at the levels of patient safety, process elements, and organizational management. At present, the use of electronic information systems, benign organizational culture and patient and family-centered clinical practice could effectively improve the efficiency and effectiveness of handover.Conclusions:The process and elements of the current shift mode are relatively complete, and the communication strategy is reasonable, but there are still many shortcomings and defects. This suggests that nursing managers should adopt scientific intervention methods and evaluation tools when paying attention to and reforming nursing handover in the future to continuously improve the quality of handover.

ABSTRACT

Objective: To study the improvement of the mental health of Japanese male registered nurses, we investigated the predictors associated with the Japanese version of the 12-item General Health Questionnaire (GHQ-12). For those predictors, we focused on environments with many female registered nurses and female patients’ refusal to accept nursing services from male registered nurses.Materials and Methods: This study was conducted in 93 hospitals in the Aichi, Gifu, and Mie prefectures of Japan. The analyzed subjects were 1,216 full-time male registered nurses. The average age (standard deviation) was 34.5 (9.2) years. Multiple logistic regression analysis was conducted. A GHQ-12 score of ≤3 indicated good mental health, and a score of ≥4 indicated poor mental health. “Mental health” was a dependent variable. The independent variables were: “Age”, “Male registered nurses as important partners on the job”, “Female registered nurses’ good manners toward male registered nurses”, “Fairness regarding male registered nurses’ promotions”, and “Female patients’ refusal to accept nursing services from male registered nurses”.Results: There were 728 (59.9%) full-time male registered nurses with good mental health and 488 (40.1%) with poor mental health. The mental health of the male registered nurses who had negative feelings regarding, “Male registered nurses as important partners on the job” was significantly worse, and that of those who had negative feelings regarding, “Female registered nurses’ good manners toward male registered nurses” was also significantly worse.Conclusion: Female registered nurses must recognize that their attitudes toward male registered nurses influence the mental health of male registered nurses. Hospital managers should provide male registered nurses who cannot establish appropriate relations with female registered nurses with consultation opportunities. Such organizational action by managers is necessary.

@#This research endeavored to explore, describe and document the saga of experienced nurse administrators to come up with reflective learning in nursing leadership and management. A qualitative phenomenological approach was utilized, specifically descriptive and interpretative methods of Husserl and Heidegger. Purposive sampling and referral were employed to select eight participants from the three fields of nursing administration(academe, hospital, and community) within Luzon, Philippines. An in-depth interview about the participants' experiences in nursing and nursing administration, together with a review of historical narratives and personal accounts were engaged to generate data. The interviews were transcribed verbatim and were analyzed following hermeneutic interpretive phenomenology. An integrated analysis of literature showed parallel descriptions of participants' responses and the context of historical accounts and events in nursing in the Philippines. Four main themes have emerged in the saga of Seasoned Nurse Administrators (SNAs): 1) Nurse administrators' responses according to the need and context of nursing in their times; 2) Nursing foundations and training are geared towards the values of nursing as a service profession, 3) The pathways towards becoming a nurse administrator are marked with competence in the roles undertaken and living the passion for service; 4) SNAs facing challenges are towards the improvement of life conditions, excellence in their profession, and setting advocacies to elevate the nursing profession in the country in the 21st century. A conceptual model was formulated in understanding the paths towards a seasoned nurse administrator.

ABSTRACT

Objetivo: Este estudo teve como objetivo propor ações relacionadas à sistematização da assistência de enfermagem e ao gerenciamento do cuidado a pacientes submetidos a cirurgias cardíacas em uma unidade de terapia intensiva. Método: Trata-se de uma pesquisa com abordagem qualitativa, desenvolvida em um hospital filantrópico, geral e de porte IV de um município do interior do Estado de Minas Gerais. Participaram do estudo os cinco enfermeiros da Unidade de Terapia Intensiva (UTI) do hospital. Os dados foram coletados no período de agosto a outubro de 2020. Para a coleta de informações relacionadas ao perfil dos pacientes submetidos a cirurgias cardíacas, foram realizadas consultas aos prontuários dos pacientes. Para analisar a percepção dos participantes, foi utilizada a técnica do Grupo Focal (GF). Os dados obtidos a partir da realização dos GF foram analisados utilizando-se a técnica da análise de conteúdo. Para a coleta de dados relacionados à caracterização dos participantes, utilizou-se um instrumento de caracterização individual e ocupacionais dos trabalhadores. Na etapa final do estudo, foram propostas ações de SAE a pacientes submetidos à cirurgia cardíaca e voltadas ao gerenciamento do cuidado de enfermagem na UTI. Resultados: Os enfermeiros reconheceram a importância de identificar as necessidades de cada paciente admitido na UTI, avaliar continuamente a evolução do paciente, implementar ações relacionadas à sistematização da assistência de enfermagem, coordenar a equipe de enfermagem e organizar a unidade para o seu adequado funcionamento. Os principais problemas e dificuldades relatados pelos enfermeiros foram: ausência de rotinas e protocolos para a padronização das ações na unidade; inadequado dimensionamento de pessoal de enfermagem na UTI; comunicação ineficaz entre a equipe de saúde; ausência de práticas de trabalho em equipe e interdisciplinaridade; falta de equipamentos para a assistência em saúde. Como produtos deste estudo, foram propostas ações voltadas à sistematização da assistência de enfermagem no pós operatório de cirurgias cardíacas e ao gerenciamento do cuidado na UTI e elaborado um fluxograma de atendimento a estes pacientes no hospital. Acredita-se que o gerenciamento do cuidado e a sistematização da assistência de enfermagem a paciente submetidos a cirurgias cardíacas possam auxiliar na efetividade das ações e na resolutividade do cuidado em saúde, garantindo autonomia no processo de tomada de decisões dos enfermeiros e qualidade na assistência de enfermagem prestada aos usuários


Objective: This study aimed to propose actions related to the nursing care systematization (NCS) and the care management for patients undergoing cardiac surgery in an intensive care unit. Method: This is a research with a qualitative approach, developed in a philanthropic, general and size IV hospital in a city in the interior of the State of Minas Gerais. Five nurses from the Intensive Care Unit (ICU) of the hospital participated in the study. Data were collected from August to October 2020. For the collection of information related to the profile of patients undergoing cardiac surgery, consultations were carried out in the patients' medical records. To analyze the participants' perception, the Focus Group (FG) technique was used. The data obtained from the FG performance were analyzed using the content analysis technique. For the collection of data related to the characterization of the participants, an instrument of individual and occupational characterization of the workers was used. In the final step of the study, NCS actions were proposed to patients undergoing cardiac surgery and aimed at managing nursing care in the ICU. Results: The nurses recognized the importance of identifying the needs of each patient admitted to the ICU, continuously evaluating the patient's evolution, implementing actions related to nursing care, coordinating the nursing team and organizing the unit for its proper functioning. The main problems and difficulties reported by nurses were: lack of routines and protocols for standardizing actions in the unit, inadequate sizing of nursing staff in the ICU, ineffective communication between the health team, lack of teamwork and interdisciplinary practices and lack of equipment for health care. As products of this study, actions aimed at the nursing care systematization in the postoperative period of cardiac surgeries and the management of care in the ICU were proposed, and a flowchart of care for these patients in the hospital was created. It is believed that the care management and the nursing care systematization for patients undergoing cardiac surgery can help in the effectiveness of actions and in the resolution of health care, ensuring autonomy for nurses' decision-making process and quality of nursing care provided to users

ABSTRACT Introduction: Leadership is the ability to guide subordinates in a direction or decision so that they can perform a task or achieve a goal that leaves them feeling empowered and accomplished. Leaders are capable of producing changes and at the same time, they inspire others to do the same. This research analysed gender-based differences and dimensions of nursing leadership styles in 18 hospitals in Andalusia, a region of southern Spain. Methods: The sample population of the study comprised 335 subjects, who were middle managers in 18 public regional hospitals in Andalusia. The instrument used to measure different leadership styles was the Multifactor Leadership Questionnaire (MLQ 5X-Short form), which is composed of 45 items. Results: The study showed that the most predominant leadership style was transactional leadership with a mean value of M = 4.22 (standard deviation [SD] = 0.42) followed by transformational leadership with a mean value of M = 3.97 (SD 0.45). Of the three styles analysed, transactional leadership had the highest statistical median for both male and female subjects. From a gender-based perspective, transformational leadership showed statistically significant differences (p < 0.01) between men and women. This was true for leadership styles as well as for the dimensions that define each style. Conclusion: The most frequent leadership style in middle nursing management in Andalusian hospitals was transactional leadership. In regard to the three leadership styles as well as their dimensions, the female subjects obtained the highest scores. This means that from a gender-based perspective, female nursing managers had better performance levels than their male counterparts in the regional hospital system in Andalusia.

ABSTRACT

Objective:To explore the application of integrated medical-nursing management model for clinical nursing teaching in neonatology department.Methods:A total of 28 undergraduate nursing students having internship in neonatology department from August 2018 to February 2020 were enrolled in the study, among which 15 nursing students from August 2018 to May 2019 were selected as the control group, and 13 nursing students from August 2019 to February 2020 were selected as the observation group. The control group was given traditional teaching, and the observation group was given integrated teaching of medical care. The theoretical scores, nursing operation, comprehensive quality, total scores and teaching satisfaction of the two groups were compared. SPSS 19.0 was used for t test. Results:The theoretical scores of the observation group and the control group were (96.31±0.85) points and (88.47±1.92) points, respectively. Nursing operation scores of observation group and control group were (95.18±2.06) points and (93.15±2.13) points. Comprehensive quality of observation group and control group were (96.23±1.48) points and (90.47±1.88) points. The total scores of the observation group and the control group were (96.83±1.27) points and (91.20±1.31) points, respectively. All the differences were statistically significant ( P<0.05). Ther were significant differences between the two groups in terms of teaching satisfaction including teaching methods, contents, teachers' satisfaction and total satisfaction( P<0.05). Conclusion:In the clinical nursing teaching of neonatology department, the integrated medical-nursing management model is helpful to improve the clinical comprehensive quality and teaching satisfaction of undergraduate nursing students.

ABSTRACT

【Objective】 To explore the effect of intelligent closed-loop mobile nursing management system in quality management of blood transfusion. 【Methods】 The mobile nursing system and the cold chain management system of our hospital were integrated to realize intelligent closed-loop management of blood transfusion. After consulting medical records, 789 blood recipients, admitted to the Department of Hematology of our hospital from January to December 2017 (before the implementation of closed-loop management), were selected as the control group, and 836 blood recipients, admitted during January to December 2018 (after the implementation of closed-loop management), were selected as the observation group. A self-designed blood transfusion standard management checklist was adopted, with the number of standard execution items as the numerator and the total number of checked items as the denominator to obtain the standard implementation rate. The checking rate of blood sample collection, blood transfusion rate within 30 min after blood issuing, the execution rate of double check specification and inspection specification, the completion rate of RBCs transfusion within 4 h and qualified rate of blood transfusion nursing record before and after the implementation of closed-loop management were compared by chi-square test or Fisher's exact probability method to evaluate the efficacy of blood quality management improvement. A total of 8 head nurses from clinical departments (Hematology Department, Cardiac Surgery Department, Operating Room, Comprehensive ICU) involved in blood transfusion were selected for semi-structured interviews. The interviews focused on the effect of intelligent closed-loop mobile nursing information system in quality management of clinical blood use, and the results of the interviews were analyzed. 【Results】 The double check execution rate of blood transfusion, the checking rate of blood sample collection specification, the transfusion rate within 30 min after blood issuing, the execution rate of blood transfusion inspection, the completion rate of RBCs transfusion within 4 h, the qualified rate of blood transfusion nursing record in the observation group and the control group after and before the implication of the intelligent closed-loop mobile nursing management system were 100%(836/836) vs 97.72%(771/789), 99.04%(828/836) vs 97.34%(768/789), 97.97%(819/836) vs 95.06%(750/789), 99.28%(830/836) vs 94.93%(712/789), 99.16%(829/836) vs 97.47%(769/789) and 100%(836/836) vs 89.73%(708/789), respectively, showing significant improvement (P < 0.05). The time spend on the record, summary and feedback of quality control results was significantly shortened. 【Conclusion】 The application of intelligent closed-loop mobile nursing management system in the quality management of clinical blood use can effectively improve the efficiency of blood transfusion, nursing quality and the safety of clinical blood use.
